Prior to drafting any contract for goods or services, the responsible party or the decedent's survivor who is handling the funeral arrangements is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent.


PROFESSIONAL SERVICES FEES

BASIC PROFESSIONAL SERVICES OF FUNERAL DIRECTOR, STAFF AND OVERHEAD

This fee is for the basic services of funeral director and staff and will be added to the total cost of the funeral arrangements you select and is non-declinable. This fee is already included in our charges for DIRECT CREMATION, IMMEDIATE BURIAL, and FORWARDING OR RECEIVING REMAINS.

This fee includes:

· Personnel available 24 hours a day, 365 days a year to respond to initial call.
· The arrangement conference.
· Coordinating funeral ceremony/service plains with clergy, cemetery, and/or other individuals involved in the final disposition of the deceased.
· Clerical assistance in the completion of various forms associated with the funeral.
· Obtaining all legal documents, certificates and permits.

TRANSFER OF DECEASED $210.00

· Additional charge per man for Transfer. $150.00

EMBALMING AND PREPARATION ROOM $595.00

Embalming is a chemical process which provides temporary preservation of the body and eliminates certain health hazards. A person licensed by the state of California must complete this procedure. Except in certain cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have a right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial.

TRI-CITY CREMATION & FUNERAL SERVICE REQUIRES EMBALMING OF THE REMAINS IF A FAMILY CHOOSES TO HOLD AN “OPEN CASKET" VISITATION AND/OR AN “OPEN CASKET" SERVICE.

DIRECT CREMATION

THIS FEE INCLUDES TRANSFER FROM THE PLACE OF DEATH IN ALAMEDA COUNTY AND BASIC SERVICE OF FUNERAL DIRECTOR AND STAFF.

If you desire to arrange a Direct Cremation, you may use an unfinished wood box or an alternative container. Alternative containers encase the body and can be made of materials like fiber board or composition materials with or without outside covering. The containers this mortuary provides for cremation is on our Casket Price List. A complete list is available at the mortuary.

YOU MAY PROVIDE YOUR OWN CREMATION CONTAINER; HOWEVER, ALL SUCH MERCHANDISE MUST MEET ANY AND ALL APPLICABLE CALIFORNIA STATE OR CEMETERY REQUIREMENTS.

Direct Cremation – purchaser provides container $985.00.

You may provide your own burial container (casket) however all such merchandise must meet any and all applicable state or cemetery requirements.
